all 5 choices are true!!
yadav- short text book of anaesthesia( 3rd edi,pg116)

stellate ganglion block
done for upper extremity sympathetic dystrophies..
blocked anterior to the tubercle of transverse process of C6 (chassaignac tubercle)
signs of successful block-
1. horner syndrome
2. flushing of face
3. conjuctival congestion
4. nasal stuffiness- guttman's sign
5. injection of tympanic membrane- mueller's syndrome
6. increased skin temperature
7. lacrimation ( if superior cervical ganglion involved)

complications-
brachial plexus block, recurrent laryngeal nerve block, phrenic nerve block.
hematoma formation, intravascular injection.
intrathecal block.
pneumothorax.
blockade of cardioaccelarator fibers.
esophageal perforation.

so, from this all options seem correct.. study
